The court will send the form, sometimes called a 鈥榩re-trial checklist鈥�, to the people or organisations involved in the case (or their legal representatives). The court will say when the form must be returned by.

There are different versions of the form for different court jurisdictions. For example:

• Form TCC / PTR1: Pre-trial review questionnaire
• Form N504: Pre-trial checklist (Directors disqualification)

Open a document

You can download and open PDF documents on your device. Downloading enables more features to be used, such as printing.

Adobe Reader is a free to use PDF viewer. You can use this to view, fill in and print PDF documents. You do not need to sign up for a free trial.

Follow these steps:

1. 聽for free.

2. Right-click on the document link then select 鈥楽ave link as鈥� or 鈥楧ownload linked file鈥�.

3. Save the form (in your 鈥榙ocuments鈥� folder, for example).

4. Open Adobe Reader and then select the saved form.
